Has the Pfizz gone from Pfizer?

Strategy plans in LatAm amidst investor pressures.

Pfizer, once a household name during the COVID-19 pandemic, is now grappling with mounting challenges. Its recent mergers and acquisitions (“M&A”) have injected concerns about financial discipline while declining revenue and growing shareholder dissatisfaction have put the company under increased scrutiny. Pfizer’s stock currently sits at USD 26.26, less than half its pandemic peak, reflecting an eye-watering USD 150 billion loss in market capitalisation.  

Activist investor Starboard Value, which has taken a substantial stake in the company, is pushing for cost-cutting measures, improved research and development (“R&D”) efficiency and a clearer path to value creation. Amid these sticky spots, Pfizer’s global expansion efforts, particularly in emerging markets, remain a critical focus. 

Although Latin America accounts for less than 20% of Pfizer’s total revenue, far below the US and other developed markets, the company has maintained an aggressive growth strategy in the region. Recent agreements with the Pan American Health Organization (“PAHO”) will facilitate the distribution of its new respiratory syncytial virus (“RSV”) vaccine, while a partnership with Sinergium Biotech and the Argentine government aims to begin local production of the pneumococcal vaccine by 2025. “An important milestone for Pfizer,” noted an acclaimed infectious disease physician.  

Starboard Value, with its USD 1 billion stake in Pfizer, has increased pressure on the company to improve R&D efficiency and implement cost discipline. However, industry sources believe this push will not uniquely impact Latin America. “I don’t see an immediate effect in the region,” the infectious disease physician stated. “Yes, Pfizer may face stricter productivity demands and strategic reviews, but current regional projects and investments are already well-advanced.” 

Pfizer’s future strategic shifts remain uncertain as Starboard Value continues to question past investment decisions. Could this prompt Pfizer to eventually move resources away from underperforming regions, potentially jeopardising its long-term expansion plans in Latin America? Our sources disagreed, “It is not affecting Pfizer’s intention to grow its presence in the region.”  

At the same time, Robert F. Kennedy Jr., known for his anti-vaccine stance, has been appointed as US Health Secretary, triggering concerns among vaccine manufacturers. While Kennedy denied being anti-vaccine in recent public hearings, his past statements have generated uncertainty. “Kennedy’s position is likely a threat to Pfizer in terms of both reputation and sales.” The physician in Brazil continued, “While he may moderate his stance in office, Pfizer will need to intensify public information campaigns and community engagement efforts.”

Given that vaccine scepticism is more prevalent in the US than Latin America, Kennedy’s appointment is unlikely to directly impact regional immunisation rates. Instead, our sources suggested that Pfizer’s biggest challenge will be maintaining strong governmental partnerships in the region to ensure continued vaccine distribution. 

The Latin American market is already experiencing a natural decline in COVID-19 vaccinations, similar to historical patterns seen with influenza vaccines, because “the sense of urgency is lost.” The pharmaceutical manager acknowledged that the anti-vaccine movement in Mexico, for example, remains relatively contained. “The larger concern is the cost, availability and government funding for vaccination programmes, as the public remains primarily focused on the accessibility to vaccines.” 

Despite shareholder pressures, Pfizer is committed to expanding into Latin America. Argentina’s partnership with Sinergium Biotech includes a USD 20 million investment to locally produce the pneumococcal vaccine, Prevenar. Industry sources noted that this is a strategic move to strengthen Pfizer’s regional footprint, “For now, Pfizer’s Latin American strategy remains intact.” The physician elaborated, “The vaccine will be produced in Argentina, which will not only serve local demand but also generate an estimated USD 100 million in annual exports.” 

Regulatory conditions in Latin America are also increasingly favourable for Pfizer’s clinical research initiatives. The senior pharmaceutical manager observed, “Pfizer sees government support for clinical research as an incentive for further investment in the region.”

While Starboard Value’s influence could lead to tighter budget controls, there is no immediate sign that Pfizer will scale back its Latin American operations. Instead, the company appears to be doubling down on strategic partnerships, leveraging regional governments and organisations like PAHO to solidify its position in the market. 

Financially, the company is still restructuring, following a 42 percent revenue decline from 2022 to 2023. Although 2024 showed a modest 7 percent recovery, Pfizer’s future in Latin America will depend on how effectively it balances shareholder expectations with its long-term regional strategy.
